Health Visitor As well as providing support and advice to parents of our under 5s, our Health Visitor carries out developmental assessments. The Health Visitor is usually available for advice and can be contacted through their office on 01292 885532, normally the answer phone is on but if you leave a message they will return your call as soon as possible. Social work – 01292 281993 General Practice Pharmacy Team We have a pharmacy team consisting of a practice pharmacist and a pharmacy technician who work within the surgery (on Monday, Tuesday and Thursday) and are there to help you to use your medicines safely and effectively by: Helping you manage long term conditions, discussing medicine concerns with you- for example, side effects, providing clinics to review your regular medicines, offering solutions if you are affected by medicine shortages, checking test results to ensure your medicines are still appropriate, liaising with health and social care partners to enhance your care.
If you would like to have a medication review, discuss your prescription or have any medicine queries please book an appointment via the practice. Community Links Practitioners Our Community Links Practitioner is able to help you work through a wide range of concerns you may have regarding keeping well in the community and is here to help you improve your health and wellbeing, through connecting you with activities and services in the area. Training, volunteering and employment. Self-management of health. Money and welfare support. Local activities and hobby groups. Housing issues. If you wish to self-refer ask the practice receptionist for further information. Hospice Nurse - Tel: 01292 269200 Our hospice nurse works with all members of our practice team and is responsible for the home care of patients referred to the Hospice. They provide help and support and are a valuable link between the hospice and the doctors in the practice in the care of our patients. Once referral has been made they can be contacted directly on 01292 269200. Community Psychiatric Nurse(CPN) Our CPN sees our patients with a wide range of conditions from anxiety and phobias to severe mental illness. They provide support within the community to our patients and provide a valuable liason with our hospital colleagues. We also have a dedicated CPN for the elderly who provides the same service but also deals with the problems unique to our elderly population. Referrals are |
